·         Proof of U.S. citizenship -- a certified birth certificate or old passport.

·         Proof of identity -- a driver’s license or state-issued photo ID.

·         Two recent identical passport photos, which may be taken on-site during the clinic for $18.

·         Cash or a check made payable to County Clerk for $25.

·         A check or a money order made payable to the U.S. Department of State for $75, if applying for a passport.

·         A check or a money order made payable to the U.S. Department of State for $20, if applying for a passport card.

A $60 fee will be charged to anyone who wishes to expedite a passport or passport card application.
